UNDERTAKING FOR SEEKING MEDICAL VISA/MEDICAL ATTENDANT VISA OF INDIA
I,_______________________(Name), holder of____________________________(Passport
Number & Nationality) has to visit India for the sole purpose of seeking medical treatment/as
medical attendant. I am a person of assured financial standing and have sufficient funds to
cover expenses related to travel, medical treatment, stay to deal with exigencies during the visit
and stay in India.
I further undertake that I would take medical treatment/discharge responsibilities of medical
attendant in the Hospital endorsed on the visa sticker and would not change the Hospital unless
it has been approved by the competent authority.
I fully understand and endorse that the medical attendant visa is granted for the purpose of
assisting the patient and thus the conduct of the medical attendant is my responsibility and so
as it is the responsibility of the medical attendant to take care of the patient in India for the
purpose of medical treatment.
I undertake to assist the local authorities/hospitals/hotels in the registration process as per the
local provisions.
I fully understand that any violation of the above provisions makes me liable for legal action as
per the laws of India.

A Duly filled Medical Visa Undertaking is a formal declaration form that an applicant must complete and sign when applying for a Medical Visa (including AYUSH Visas) to India. It confirms that the applicant:

  1. Is traveling to India specifically for medical treatment—in this case, under AYUSH or traditional Indian medical systems.

  2. Will abide by Indian laws and the conditions of the visa.

  3. Takes full responsibility for the authenticity of the submitted medical documents.

  4. Will not overstay the visa duration or misuse it for purposes other than stated.

  5. Will report to the hospital/institution mentioned in the application and notify authorities if treatment plans change.

Duly filled Medical Visa Undertaking form requires:

  • Applicant’s personal and passport details

  • Name of the hospital or AYUSH institution in India

  • Type of treatment to be received

  • Duration of intended stay

  • Declaration of financial responsibility for the treatment and stay
